Qualifications

• Active, unrestricted Registered Nurse license with no disciplinary action.
• 5+ years of RN experience.
• 3+ years of experience in case management.
• 1+ year of data-driven leadership experience, including analyzing performance and implementing improvements.
• Strong proficiency in Microsoft Office and ability to learn multiple software applications.

Responsibilities

• Provide leadership and coaching for associates conducting telephonic, Stars-focused member outreach.
• Ensure teams identify needs beyond outreach and refer to clinical, social, or community resources.
• Promote an engaged, inclusive team and maintain accountability across productivity and care-gap closure.
• Translate Stars priorities into team actions and monitor performance to improve outcomes.
• Collaborate with Enterprise Stars, quality, operations, analytics and other teams to improve performance.
