Job summary

You will document assessments, develop care plans with the care team, participate in care rounds, and assist patients through transitions while maintaining HIPAA standards.

Qualifications

• RN required with valid license and good standing.
• Experience in care coordination or case management preferred.
• Strong communication and documentation skills.
• Ability to work with high-risk and Medicaid populations preferred.
• Proven ability to coordinate care across multiple teams.
• Familiarity with HIPAA and confidentiality rules.

Responsibilities

• Document member information and interventions in care management systems.
• Develop and implement care plans in collaboration with members and care teams.
• Participate in care rounds and share member reports with the team.
• Identify high-risk members and coordinate with providers to navigate care.
