Job summary

Alpine is seeking a Care Manager (RN) to deliver clinically and psychosocially informed, person‑centered care management to Medicaid members with complex needs. Responsibilities include comprehensive assessments, care planning, transitions of care, and coordination with PCPs, behavioral health providers, and community resources.

The RN must be licensed in the served state and bring 1+ year of care management or related experience; bilingual ability is a plus and telework is supported where

Qualifications

• Active unrestricted RN license in the state served.
• 1+ year experience in care management or related field.
• Experience with Medicaid/DSNP/CSNP preferred.
• Bilingual ability considered a plus where relevant.

Responsibilities

• Perform comprehensive clinical and psychosocial assessments for high-risk, medically complex members.
• Develop, implement and update individualized care plans addressing medical, behavioral health, psychosocial, and environmental needs.
• Manage transitions of care following hospitalization or acute episodes.
• Coordinate care with PCPs, specialists, behavioral health providers, and community resources.
• Maintain timely, accurate documentation and HIPAA compliance.
