Job summary

Bluestone is seeking a Care Coordinator to manage essential activities for Minnesota Senior Health Options members. The role combines remote work and in-person visits, ensuring high-quality care for clients in assisted living and the community.

The ideal candidate will have a RN license or be a licensed social worker in Minnesota, and experience with the elderly or disabled populations is preferred. This position offers a salary of $65,000 – $75,000 and a comprehensive benefits package.

Qualifications

• Must have a Minnesota Registered Nurse license or be a Licensed Social Worker.
• Experience with the geriatric or certified disabled population is preferred.
• Access to a private and compliant home office is required.

Responsibilities

• Coordinate face-to-face visits with clients.
• Manage Elderly Waiver and conduct annual assessments.
• Develop individualized care plans and connect with resources.
