Job summary

Bluestone is hiring a Care Coordinator to work with Minnesota seniors in community and assisted living settings. You’ll travel to members’ homes 50–70% and perform DHS/CMS required activities for MSHO/MSC+ with a focus on quality care and outcomes.

Full-time position with day hours, travel-heavy in the Winona area and remote work portions. Requires MN RN or MN SW licensure and driving benefits; experience with geriatric populations is preferred.

Qualifications

• Minnesota RN license or MN Social Work license (new grads encouraged).
• Experience with geriatric or disabled populations in case management/care coordination preferred.
• Must have a valid driver’s license.

Responsibilities

• Coordinate face-to-face visits with community members.
• Manage the Elderly Waiver program.
• Conduct annual PCA assessments and develop personalized living tools.
• Review health needs, set goals, and develop care plans.
• Link members with community and state resources and services.
• Complete required documentation and ensure quality measures.
• Support care transitions and safe discharges with the team.
