Job summary

Bluestone is seeking a Care Coordinator to support Minnesota seniors living in the community and assisted living facilities. You will work from home, traveling regularly in your local area, to deliver DHS and CMS-required activities for MSHO/MSC+ members.

Schedule is full-time, day shift with no evenings or weekends. Location requires 50-70% travel in the Winona, MN area and 30-50% remote work; salary ranges from $65,000 to $75,000 with a $2,500 sign-on bonus.

Qualifications

• Must hold Minnesota RN license or MN SW license.
• Experience in geriatric or disabled population case management preferred.
• Must have valid driver’s license.

Responsibilities

• Coordinate face-to-face visits with senior members.
• Manage Elderly Waiver and coordinated services.
• Lead annual PCA assessments and living tools for members.
• Develop individualized care plans based on health needs.
• Connect members with community and state resources.
