Job summary

Bluestone is seeking a Care Coordinator to manage DHS and CMS activities for Minnesota Senior Health Options members living in the community and assisted living. You will work from home with regular local travel to support members and the community.

This full-time role involves day hours, 50-70% travel around Little Canada and Mahtomedi areas with 30-50% remote work; salary ranges from $65,000 to $75,000 and includes a sign-on bonus after milestones.

Qualifications

• RN license in Minnesota or active MN social worker license.
• Minimum 1 year geriatric/care coordination experience preferred.
• Valid driver\'s license required.

Responsibilities

• Coordinate face-to-face visits with members.
• Manage Elderly Waiver and PCA assessments.
• Develop individualized care plans and goals for members.
• Connect members with community/state resources and services.
• Document required information accurately.
• Collaborate with medical teams to meet quality measures.
• Support transitions of care and safe discharge planning.
