Job summary

The Coordinating Center is seeking a Clinical Care Coordinator to provide day-to-day care management for clients in the Model Waiver Program. You will partner with families to create individualized plans and coordinate services across medical and community supports in a hybrid telework and field visit role.

Ideal candidates hold an active Maryland RN license with at least two years in case management; pediatric experience is preferred.

Qualifications

• Active RN license in Maryland.
• Two years of case management experience.
• Pediatric experience preferred.
• CCM certification within two years of employment.

Responsibilities

• Develop and coordinate individualized care management plans with client and family input.
• Coordinate medical and home/community-based services for medically complex clients.
• Navigate cost-effective care alternatives and document all activities in electronic systems.
• Maintain confidentiality of PHI and ensure quality care delivery.
• Attend multidisciplinary team meetings and conduct periodic client contact.
