Job summary

Eterna Primary Care is seeking a Call Center Nurse/Agent to serve as the first point of contact for patients, families, SNFs, ALFs, and providers. You will manage high-volume calls, document interactions, triage requests, and coordinate with medical staff to resolve patient needs.

The role offers hybrid work with onsite at the support center or remote options, including occasional travel for company-wide meetings and performance improvement initiatives.

Qualifications

• Associate degree or higher required or equivalent experience.
• RN license with multi-state eligibility preferred.
• Experience in skilled nursing/post-acute care is preferred.

Responsibilities

• Answer incoming calls in a professional, courteous manner.
• Document all encounters accurately in the EMR.
• Coordinate same-day provider visits and follow-ups.
• Maintain patient confidentiality and HIPAA compliance.
