Eterna Primary Care is seeking a Call Center Nurse to serve as the first point of contact for patients, families, SNFs, ALFs, hospitals, pharmacies, and providers. You will manage high-volume calls, document communications, triage requests, and coordinate with medical staff to ensure timely care needs.

The role supports continuity of care and aims to improve patient satisfaction while reducing hospitalizations.

Qualifications

• Associate degree or higher required or preferred.
• Current and active RN license preferred; multi-state license may be advantageous.
• Experience with healthcare terminology and care coordination is beneficial.

Responsibilities

• Answer incoming calls professionally and compassionately.
• Document calls accurately in EMR or designated system.
• Triage requests and coordinate with medical staff for timely care.
