Job summary

Oak Street Health is seeking a Clinical Call Center Triage Nurse to extend care to patients telephonically outside regular clinic hours. You will address medical, social, and psychological needs, and document thoroughly on calls.

Qualifications

• Active RN license in all OSH practice states.
• Proactive maintenance of licenses without prompt.
• Ability to manage time and work autonomously with consistency.
• Focus on delivering an unmatched patient experience on every call.
• Willingness to work flexible shifts including evenings, weekends, and holidays.

Responsibilities

• Provide confident and professional service to OSH patients.
• Follow current Prescription Refill policy.
• Document clearly and completely during calls.
• Complete documentation for all patient requests prior to ending a call.
• Check and complete CareReqs as assigned.
• Schedule appointments for off-cadence or requested visits.
• Arrange transportation for clinic visits.
• Demonstrate reliability and meet departmental metrics.
• Perform other duties as assigned.
