Job summary

Community Health Network, Inc. is seeking a Remote Risk Adjustment Coder to review patient charts, ensure outpatient HCC coding accuracy, and support CMS compliance from home. Reporting to the CDI Manager, you will use various platforms to complete workflows across eligible states.

The role requires 3+ years in outpatient risk adjustment and coding, with preferences for population health experience and relevant certifications. This is a remote role with growth opportunities within the CDI team.

Qualifications

• Three (3) or more years of experience in professional OUTPATIENT Risk Adjustment (HCC) coding.
• Three (3) or more years of experience in population health, VBC/ACO is preferred.
• Three (3) or more years of experience in OUTPATIENT Coding is preferred.
• Must obtain one of the following certifications through AAPC and/or AHIMA within 6 months from hire (CPC, CPC-H, CPC-I, CPC-A, CCSP, CCS, CRC).

Responsibilities

• Timely, accurate review of patient charts for CMS HCC-based diagnosis coding.
• Utilizing multiple technical platforms to complete CDI workflows.
• Validating diagnosis codes with MEAT documentation and regulatory guidelines.
• Conducting audits to meet ACA standards and compliance.
• Creating post-visit queries and collaborating with CDI team members.
