Job summary

UASI is seeking a Risk Adjustment Clinical Documentation Specialist to advance documentation accuracy in a fully remote capacity. The role focuses on supporting coding accuracy, HCC capture, regulatory compliance, and clinical documentation integrity in outpatient settings.

You will review medical records, use Epic and OPAs for prospective chart reviews, manage work queues, analyze CDI outcomes, and contribute to program goals with strong clinical judgment and communication skills.

Qualifications

Minimum 2 years of ambulatory Clinical Documentation Integrity or related healthcare documentation experience.
Active LPN or RN license required.
CCDS, CCDS-O, CDIP, CDEO and/or CRC certification strongly preferred.
Proficiency in Epic workflows and OPAs encouraged.

Responsibilities

Serves as a subject matter expert in clinical documentation quality, risk adjustment, and HCC coding integrity in the outpatient setting.
Performs prospective reviews of medical records to validate chronic conditions and ensure documentation accuracy and coding.
Utilizes Epic and OPAs to perform chart reviews and support accurate coding and HCC capture.
Track and manage work queues, review inventories, and ensure timely CDI interventions.
Monitor, analyze, and report CDI outcomes and HCC recapture performance to identify improvements.
