Job summary

Evergreen Healthcare Group is seeking a Regional Central Intake Director to manage SNF referrals across designated regions. This part-time, fully remote role handles referrals, coordinates with clinical teams and facility leadership, and enters data into the PCC CRM following Green/Yellow/Red guidelines.

The ideal candidate has 3–5 years in SNF admissions or related fields, strong communication, multitasking, and knowledge of Medicare/Medicaid processes.

Qualifications

• Strong knowledge of SNF admissions and referral management.
• Understand medical terminology and clinical capability grids (Green/Yellow/Red).
• Able to multi-task in a fast-paced environment.
• Excellent verbal and written English communication.
• Experience with Medicare, Medicaid, ManagedCare.
• Self-starter with time management and problem-solving skills.

Responsibilities

• Respond to referrals via portals within 30 minutes or less.
• Perform medication cost-outs for referrals.
• Route yellow referrals to clinical leadership for evaluation.
• Enter referrals into PCC CRM accurately.
• Collaborate with facility clinical teams for smooth admission transitions.
• Communicate acceptances and denials with hospital personnel.
• Keep up-to-date with SNF admission regulations and best practices.
