Qualifications

• Residence must be within Indianapolis, Indiana area.
• Must have valid Indiana driver's license and proof of insurance.
• High School Diploma or GED.
• 1–3 years of health services or behavioral health experience in community-based settings.
• Knowledge of community-based populations, medical conditions, and disabilities.
• Must be fully vaccinated against COVID-19 and flu vaccine.

Responsibilities

• Coordinate follow-up care plans by scheduling appointments or enrolling members in programs.
• Provide telephone and/or on-site recovery support.
• Make referrals to the Case Management Services.
• Assess adherence to medical treatment plans via telephone or home visits.
• Identify barriers to plan compliance and coordinate resolutions.
• Assist accessing community resources (education, housing, transportation, recovery supports).
• Coordinate referrals to local, state or federally funded programs.
• Participate in cross-functional teams on projects and process improvements.
• Maintain confidentiality of sensitive information and ensure HIPAA compliance.
