Job summary

Engaging Solutions is seeking a Community Health Worker to serve as the primary contact for members enrolled in the Indiana PathWays for Aging program. You will conduct home visits and telephonic assessments to screen health needs, arrange referrals, and distribute materials.

Responsibilities include coordinating care plans, facilitating access to community resources, and collaborating with case management and providers to adjust treatment plans as needed.

Qualifications

• High School Diploma or GED required.
• 1-3 years in health and human services, public health, Medicaid/Medicare, or related field with diverse community exposure.
• Bilingual candidates preferred.
• Valid Indiana driver’s license, proof of insurance, access to reliable transportation.
• Ability to work from home with dedicated space.

Responsibilities

• Coordinate follow-up care plan needs by scheduling appointments or enrolling members in programs.
• Support member access to care and provide telephonic/onsite assistance.
• Refer to Case Management Services as needed.
• Assess adherence to medical plans via calls or home visits.
• Identify barriers and coordinate resolutions with internal teams.
