Qualifications

• Ability to complete 35 community visits per week.
• Strong interpersonal and communication skills.

• Must be vaccinated for COVID-19 and flu.
• Ability to obtain Indiana driver’s license and reliable transportation.

Responsibilities

• Coordinate follow-up care plans by scheduling appointments or enrolling members in programs.
• Provide telephone and onsite recovery support.
• Refer members to Case Management Services.
• Assess compliance with medical treatment plans via phone or in-person visits.
• Identify barriers to plan compliance and coordinate resolutions.
• Assist accessing community resources (education, housing, transportation, recovery supports).
• Recommend treatment plan modifications in collaboration with case management and providers.
• Coordinate referrals to local, state or federally funded programs.
• Participate in cross-functional teams on projects and process improvements.
