Job summary

Engaging Solutions, LLC in Indiana is seeking a Certified Community Health Worker to serve as the primary liaison between members and our health programs. You will conduct health screenings, assess needs, refer members to resources, and support access to care, primarily through home visits and community outreach.

Applicants should have a High School diploma or GED, CHW certification, and the ability to work independently while coordinating with a care team.

Qualifications

• Minimum high school diploma or GED; CHW certification preferred.
• Ability to obtain a valid Indiana driver's license and access to reliable transportation.
• Experience working with diverse community populations is desirable.

Responsibilities

• Coordinate follow-up care plan needs by scheduling appointments or enrolling members in programs.
• Provide telephone and/or on-site recovery support to members.
• Refer members to Case Management Services and assist with accessing community resources.
• Assess member compliance with treatment plans via telephone or home visits.
• Identify barriers to plan compliance and coordinate resolutions with internal teams.
