Job summary

Engaging Solutions is seeking a Certified Community Health Worker in Franklin, IN to serve as the primary contact for members, conduct health screenings, and coordinate access to care. The role includes home visits, referrals to case management, and distribution of resources to support health outcomes.

Responsibilities include scheduling follow-up plans, providing recovery support, and ensuring compliance with HIPAA while working from home when not in the community. Indiana residency is required.

Responsibilities
