Job summary

Duly Health and Care is seeking a Quality Improvement Nurse to support initiatives that improve clinical outcomes and patient care delivery. You will collaborate with providers, clinic staff, and leadership to identify care gaps, improve medication adherence, and support quality programs.

The role requires strong Excel skills, an RN license in good standing, and experience in primary care or population health. Regular travel to clinical locations and monthly HQ presence are expected.

Qualifications

• Active RN license in good standing.
• Current CPR (BLS) certification.
• Minimum 2+ years of RN experience in primary care, care management, population health, or quality improvement.
• Strong analytical skills with advanced proficiency in Microsoft Excel.

Responsibilities

• Analyze payer, clinical, and operational data to identify quality gaps and patients requiring outreach or intervention.
• Utilize advanced Excel to analyze datasets, track performance, build reports, and monitor initiative progress.
• Conduct patient outreach to support medication adherence, care gap closure, and programs.
• Identify barriers to adherence and care gap closure, and elevate findings to providers and care teams.
• Work closely with providers and clinic staff to close quality gaps through workflow optimization and outreach coordination.
