Qualifications

• Master's Degree required in a relevant clinical field.
• Board-certified APRN or PA-C required.
• Active DEA and New Hampshire license required.
• Minimum 3-5 years of clinical experience in primary care.

Responsibilities

• Support primary care teams remotely by completing indirect patient care tasks as an APC Remote Primary Care InBox provider.
• Collaborate with on-site providers and staff to co-manage the medical needs of patients.
• Respond to patient calls and portal messages requiring provider input.
• Review and process medication refill requests.
• Coordinate patient care with triage and care coordination nurses.
• Communicate lab and diagnostic test results to patients.
• Complete peer-to-peer reviews with insurance companies for authorization of testing or treatment when needed.
• Attend regularly scheduled virtual meetings with on-site providers.
