Job summary

Advocate Health in Charlotte, NC seeks an Advanced Practice Registered Nurse (APRN) or Physician Assistant (PA) to deliver care via virtual health and in-person visits. You will assess, diagnose, and manage treatment plans, order diagnostics, and coordinate with an interdisciplinary health team.

Experience with chronic disease management and remote monitoring is a plus. Responsibilities include EMR documentation, adherence to evidence-based care, patient education, and participation in quality

Qualifications

• Requires APRN with active RN and board certification (Family/Adult/Adult-Gero).
• Or PA with active license and NCCPA certification.
• Active DEA registration.
• Illinois license if Illinois practice.

Responsibilities

• Assess, diagnose, and manage treatment via virtual health and in-person visits.
• Order, perform, and interpret diagnostic studies; prescribe interventions.
• Collaborate with interdisciplinary team; escalate care when needed.
• Maintain accurate EMR documentation and compliant coding/billing.
• Participate in quality, safety and peer review initiatives and meetings.
• Educate patients and families; onboard new team members; pursue ongoing skill development.
