Remote Care Partners is seeking a compassionate Nurse Practitioner to join our growing virtual care team. This is a fully remote role requiring an active APRN license in SC, VA, or GA, with additional state licenses encouraged.

You will deliver high-quality primary care via telehealth, oversee RPM/CCM programs, and support weight management initiatives. Join a technology-driven, patient-centered environment.

Qualifikationen

• Active, unrestricted APRN license in SC/VA/GA.
• 3+ years NP experience in primary care or similar.
• Experience with RPM/CCM and telehealth preferred.
• Ability to work independently and with a team.

• Conduct virtual primary care visits for adults.
• Perform RPM and CCM visits and oversee programs.
• Conduct weight management consultations and follow-ups.
• Document care in EHR and ensure Medicare/compliance adherence.
• Collaborate with care team to optimize outcomes.
