Job summary

Devoted Health is seeking an experienced Primary Care Advanced Practice Provider (APRN or PA) to deliver longitudinal virtual primary care to a panel of patients through our telehealth platform. You’ll conduct comprehensive assessments, develop plans for chronic condition management, and collaborate with a multidisciplinary team to provide patient-centered care.

Active license as an APRN or PA with board certification in Family Medicine, Adult-Gerontology, or Internal Medicine is required, with

Qualifications

• Active Advanced Practice Provider license and board certification in Family Medicine, Adult-Gerontology, or Internal Medicine.
• Minimum 4 years of primary care or related experience, with telehealth exposure preferred.
• Experience managing chronic conditions and preventive care via telehealth.
• Familiarity with value-based care models and quality metrics is a plus.
• Comfort with digital documentation and telemedicine platforms.

Responsibilities

• Deliver longitudinal virtual primary care visits via telehealth.
• Anchor an interdisciplinary primary care team to deliver coordinated care.
• Diagnose and manage chronic conditions and develop individualized care plans.
• Order and interpret labs/imaging and coordinate follow-up.
• Manage medications including titration and refills with safety.
