Job summary

UnitedHealthcare is seeking a Senior Clinical Transformation Consultant to lead multi-disciplinary practice transformation within ACO/Population Health programs. You will guide physician practices toward quality, efficiency, and cost goals with data-driven strategies.

You will mentor Transformation Consultants, communicate progress to regional leadership, and align care delivery with reporting tools, registries, and the Physician Portal.

Qualifications

• Bachelor's degree or 9+ years of experience in clinical quality, analytics, or transformation.
• Experience driving population health and care transformation initiatives.

Responsibilities

• Deploy and manage clinical transformation programs across practices.
• Lead multidisciplinary teams to meet quality, utilization and savings goals.
• Mentor other Transformation Consultants and report progress to leadership.
• Use data and reporting tools to monitor milestones and outcomes.
• Collaborate with care teams to integrate technology into workflows.
• Support practice performance improvement using rapid-cycle methods (PDSA).
