Job summary

Aledade is seeking a Practice Transformation Specialist to support New England practices and improve patient care through value-based care initiatives. You will work with providers to implement wellness visits, transitions of care, and care management, with travel roughly monthly for on-site checks.

This remote-first role requires residency in New England, strong communication and project management skills, and experience with Electronic Health Records.

Qualifications

• Graduate of an accredited college or university.
• 4-6 years of significant and relevant work experience in medical practice management may be accepted in lieu of educational requirements.
• 3 years of experience with EHR for clinical/practice management processes.

Responsibilities

• Direct Practice Support: implement products and execute ACO initiatives; ensure wellness visits, transitions of care, and active management meet goals.
• ACO Training & Development: train office staff and providers on processes and technologies; assess training needs.
• Customer Success: listen to practices' concerns and relay opportunities for improvement.
