Job summary

Allegheny Health Network is seeking a referrals coordinator to facilitate and monitor the post-acute care referral workflow from the acute care setting to home. You will work with the Transitional Care Coordinator to coordinate the patient’s needs, ensuring timely, accurate compliance with regulations and documentation.

The role requires strong computer skills, project coordination, and the ability to work with internal teams and external partners.

Qualifications

• High school diploma or GED is required.
• Clinical or medical background.
• 1 year experience with Electronic Medical Records (EMR).
• Associates degree preferred.
• 1-3 years experience in the medical profession preferred.
• 1 year of Customer Service preferred.

Responsibilities

• Process referrals for post-acute care through established workflows.
• Confirm referrals and escalate barriers to ensure service delivery.
• Engage patients to obtain post-acute care choices.
• Build and maintain relationships with post-acute and hospital partners.
• Respond to consumer inquiries about post-acute services.
• Participate in on-site rotations and process improvement initiatives.
