Responsibilities

• Serves as the primary contact for referring physicians, patients, caregivers, families, and the community.
• Facilitates patient flow and coordinates appointments within the network.
• Coordinates care with the multidisciplinary team for adult patients with specialized disease states.
• May assist Registry staff with data collection and care delivery model updates.
• Ensures medication adherence and coordinates multi-disciplinary appointments.
