Presbyterian Healthcare Services is seeking a Remote Clinical Program and Population Health Strategist-Commercial to advance population health strategy for Commercial populations and translate enterprise priorities into program designs within our IDS framework.

You will partner across health plan, medical group, and care delivery to prioritize Clinical Initiatives, define success metrics, and support scalable implementation with a focus on HEDIS performance and cost management.

Formación

• Bachelor’s degree required; Master’s degree preferred.
• 5–10+ years of experience in population health, healthcare strategy, or clinical program design.
• Experience with Commercial populations and population-health priorities.

Responsabilidades

• Advance population health strategy for Commercial populations within the IDS model.
• Translate enterprise priorities into product-specific clinical initiatives and ROI-focused programs.
• Partner with analytics, product owners and leadership to define success metrics (HEDIS, affordability, utilization).
• Design evidence-based clinical programs and interventions targeting specific populations.
