Presbyterian Healthcare Services is seeking a Clinical Program & Population Health Strategist to advance Government Programs initiatives remotely. The role focuses on ACA, Medicaid, and D-SNP populations, with a full-time, exempt status and NM remote base.

The position supports health value strategies across payer and care delivery systems. The strategist translates enterprise priorities into program designs, prioritizes initiatives for ROI, defines success metrics (HEDIS/Stars), and partners

Qualifications

• Bachelor’s degree required; Master’s degree preferred.
• 5–10+ years in population health, healthcare strategy, or clinical program design.
• Experience with Medicare (D-SNP) and/or Medicaid; strong HEDIS/Stars knowledge.

Responsibilities

• Develop population health strategies for ACA, Medicaid, and D-SNP populations.
• Translate enterprise priorities into clinical program designs and ROI-focused initiatives.
• Partner with product owners, analytics, and operations to drive implementation.
• Define success metrics aligned with HEDIS, Stars, risk adjustment, and utilization goals.
• Inform build vs. buy decisions for clinical capabilities and provider tools.
