Job summary

You will translate enterprise priorities into clinical program designs, define target populations, and partner across the health plan, medical group, and care delivery system to drive measurable outcomes through digital, virtual, and in-person care models.

Qualifications

• Bachelor’s degree required; MPH/MHA/MBA or related preferred.
• 5–10+ years in population health or healthcare strategy.
• Experience with Commercial populations.
• Strong understanding of HEDIS, cost drivers, and utilization.
• Experience in a matrixed organization.
• Strategic thinking and portfolio prioritization.

Responsibilities

• Develop commercial population health strategy aligned with enterprise priorities.
• Identify high-impact opportunities to improve outcomes in preventive care and chronic disease.
• Define success metrics aligned with HEDIS and affordability.
• Design clinical programs and interventions with target populations.
• Partner with product owners and leadership to prioritize initiatives.
• Support implementation and scaling with operational leaders.
• Collaborate with analytics, network, sales, and care management.
• Inform digital, virtual, and in-person care models.
