Job summary

Atrius Health is seeking a Population Management Care Facilitator to support population health outreach and chronic disease management for primary care patients. The role requires organizing outreach efforts, coordinating with the care team, and ensuring adherence to planned care across multiple clinical teams.

Work is primarily remote with quarterly on-site meetings at the Auburndale location. The target candidate will navigate quality metrics, onboard patients, and assist in data-driven care

Qualifications

• Requires a high school diploma or equivalent.
• Experience in clinical or ambulatory care settings is preferred.
• Must reside within New England and be able to attend quarterly on-site meetings.

Responsibilities

• Organize and coordinate outreach efforts for population management at the site.
• Review charts and identify care gaps to recommend interventions.
• Communicate with clinicians, staff, and external partners to relay clinical information.
