Job summary

Upperline Health Plus is seeking a full-time Remote Pharmacy Technician to support our clinical pharmacy program. You will conduct telephonic patient outreach, gather medication histories, and support adherence initiatives in a fully remote, Monday–Friday schedule with no dispensing or on-site duties.

Under supervision of a licensed Pharmacist, you will collaborate with the clinical team on reconciliation, outreach, and quality measures, while ensuring HIPAA compliance in a remote work

Qualifications

• Active California pharmacy technician registration in good standing.
• Minimum of three (3) years of pharmacy technician experience.
• Strong understanding of pharmacy terminology, prescription medications, OTC products, and dietary supplements.
• Excellent attention to detail and accurate documentation.
• Comfort spending most of the day on the phone with patients, prescribers, and payers.
• Exceptional customer service with empathy and professionalism.

Responsibilities

• Conduct outbound phone outreach to obtain and verify medication histories.
• Perform medication reconciliation and escalate questions to the supervising Pharmacist.
• Promote medication adherence through refill reminders and 90-day prescription coordination.
• Document outcomes in the EHR and care management platform.
• Identify copay assistance and affordability programs for review.
• Coordinate with care teams and pharmacies to resolve access barriers.
• Maintain HIPAA-compliant, timely documentation in the EHR.
• Support remote workflow and complete other duties as assigned.
