PharMerica is seeking a Pharmacist for a full-time, remote role serving long‑term care and senior living clients. Work hours align with Central Time Zone; schedule is Monday–Friday, 11am–7:30pm CT. Licensed pharmacist in the state of practice; 1+ year LTC experience preferred.

Formación

• Bachelor’s degree in Pharmacy required; Pharm D preferred.
• One year experience in long term care desired.
• Current pharmacist license in state of practice.
• Knowledge of CMS/DEA/FDA guidelines and medication safety practices.
• Proficiency with electronic medical records and pharmacy systems.
• Strong clinical judgement and communication skills.

Responsabilidades

• Evaluate physician orders for drug appropriateness, dosage, interactions, and administration route.
• Provide clinical recommendations to physicians and staff on therapy changes.
• Maintain resident profiles with history, labs, and medications.
• Order and review labs for high‑risk meds and communicate with the team.
• Collaborate with physicians, nurses and staff to support patient care.
• Document interventions in the medical record and communicate with families.
• Enforce policies for safe dispensing and medication management; ensure regulatory compliance.
