Qualifications

• Current license required; dual license preferred.
• 1+ years of experience in acute, long-term care or home health preferred.
• Good understanding of formulary management.
• Ability to stand for long periods and lift patient charts.

Responsibilities

• Evaluate physician medication orders for appropriateness.
• Review drug regimen and clinical indicators monthly.
• Document communication with healthcare team members.
