PharMerica, an affiliate of BrightSpring Health Services, is hiring a full-time Pharmacist for remote work serving long-term care and senior living clients. The role emphasizes clinical collaboration, safety, and regulatory compliance while supporting a distributed, centralized pharmacy team.

您 will evaluate orders, provide therapy recommendations, maintain resident profiles, and ensure appropriate monitoring for high-risk medications. Requires active pharmacist license and LTC experience.

Qualifications

• Bachelor’s degree in Pharmacy required or PharmD preferred.
• One year of experience in long-term care is desirable.
• Current pharmacist license in the state of practice is required.
• Knowledge of drug therapy guidelines, medication safety practices, and regulatory standards (CMS, DEA, FDA).
• Proficiency with electronic medical records and pharmacy information systems.
• Strong clinical judgement and effective written and verbal communication.

Responsibilities

• Evaluate physician medication orders for drug appropriateness, correct dosage, potential interactions, and proper administration route.
• Provide clinical recommendations to physicians and healthcare staff regarding medication therapy changes and optimizations.
• Maintain up-to-date patient/resident profiles including medical history, lab results, and current medications (prescription, OTC, home remedies).
• Ensure laboratory tests required for monitoring high-risk medications are ordered, reviewed, and communicated to the healthcare team.
• Collaborate with physicians, nurses, and other healthcare professionals to provide clinical and technical support.
• Document all communications and interventions in the patient’s medical record, including interactions with families and caregivers.
• Implement and enforce policies for safe handling, dispensing, and clinical oversight/management of medications.
• Ensure compliance with state and federal regulatory requirements related to pharmacy practice and medication management.
• Report medication-related errors and adverse drug events to the Pharmacy Manager as required.
• Educate and support clinical team and healthcare agency staff on safe and effective medication handling procedures.
• Perform other pharmacy-related tasks and responsibilities as assigned.
