Job summary

PharMerica is seeking a Pharmacist for our long‑term care and senior living clients in Texas. You will work in a non-retail, closed‑door setting, reviewing orders, advising on therapy changes, and maintaining resident medication profiles to ensure safe, effective care.

The role emphasizes collaboration with physicians and the healthcare team, compliance with CMS/DEA/FDA standards, and ongoing professional development in a growing national pharmacy network.

Qualifications

• Bachelor’s Degree in Pharmacy; Pharm D preferred.
• One year of experience in long-term care desired.
• Current and valid pharmacist license in the state of practice.
• Knowledge of drug therapy guidelines, medication safety practices, and regulatory standards (CMS, DEA, FDA).
• Proficiency in electronic medical records and pharmacy information systems.

Responsibilities

• Evaluate physician medication orders for drug appropriateness, correct dosage, potential drug interactions, and proper route of administration.
• Provide clinical recommendations to physicians and healthcare staff regarding medication therapy changes and optimizations.
• Maintain up-to-date patient/resident profiles with medical history, labs, and current medications.
• Ensure laboratory tests required for monitoring high-risk medications are ordered, reviewed, and communicated to the team.
• Collaborate with physicians, nurses, and other healthcare professionals to provide clinical and technical support.
