Qualifications

High school diploma or equivalent; additional degrees preferred.
Certification through the Missouri Credentialing Board required within 2 months.
Community Health Worker certification within one year if requested by supervisor.
1–2 years in a community-based setting preferred.
Valid driver’s license and proof of insurance for use of a personal vehicle.

Responsibilities

Carry out NHS goals with cooperation from providers and staff.
Conduct intake interviews and assess social determinants of health.
Follow up with patients via phone, home visits, school visits, or clinic visits.
Assist patients with applications and registrations; refer to community resources.
Maintain patient care reports and communicate progress to the care team.
Attend staff trainings and meetings; perform other duties as assigned.
