Qualifications

Responsibilities

• Conduct intake interviews, assess social determinants of health and barriers to treatment; develop patient-centered plans.
• Follow up with patients via phone calls, home visits, school visits, or clinic visits.
• Assist patients with completing applications and registration forms.
• Provide education and referrals to community resources as needed.
• Record patient care information in the EMR within 24 hours of contact.
• Manage assigned caseload and attend regular staff meetings and trainings.
