Trillium Health Resources is a Tailored Plan and MCO serving 46 counties in North Carolina. We seek a Peer Support Specialist to assist individuals navigating care transitions and to promote recovery and community integration.

The role uses lived experience to build trust, connects people to resources, and supports engagement in services. Remote within the Southern Region with travel as needed.

Qualifikationen

• High School Diploma or GED.
• Year of lived experience with a behavioral health condition and/or substance use.
• North Carolina Peer Support Specialist Certification.
• Valid driver’s license required.
• Must reside in Trillium's Southern Region (Bladen, Brunswick, Columbus, Hoke, Lee, Moore, New Hanover, Robeson, Scotland).
• Must be able to travel within catchment as required.

Aufgaben

• Provide peer-based support using lived experience to build trust and engagement in services.
• Support outreach and engagement to connect individuals to services and community resources.
• Assist individuals in navigating services and accessing supports.
• Facilitate group activities, educational sessions, and community engagement opportunities.
• Promote self-advocacy, independence, and active participation in services.
