Job summary

Blue Ridge Health is seeking a Peer Extender to join our Tailored Care Management team in Clyde, NC. The role focuses on recovery support within an interdisciplinary team delivering holistic care that integrates physical, behavioral health, and community services.

You will engage members, coordinate services, promote health activities, and assist in referrals and resource access, contributing to person-centered care delivery. This is a full-time, on-site position offering competitive benefits.

Qualifications

• High school diploma or equivalent required.
• Certified Peer Support Specialist with lived behavioral health experience.
• Driver's License required.
• Previous Peer Support Specialist (PSS), Community Health Worker (CHW), Community Navigators or Family Navigators a plus.

Responsibilities

• Performing general outreach, engagement, and follow-up with members.
• Coordinating services/appointments (e.g., reminders, transportation).
• Engaging in health promotion activities as defined in Tailored Care Management Provider Manual.
• Sharing information with the care manager and other team members on member circumstances.
• Providing and tracking referrals and assisting in obtaining community resources.
• Participating in case conferences.
• Support the care manager in assessing and addressing unmet health-related resources.
