Holon Health is seeking a Recovery Coach/PRS in Wisconsin for a remote, full-time position. You will support justice-involved individuals on their recovery journey, offering encouragement, building trust, and helping navigate barriers to lasting wellness.

Your lived recovery experience and PRS certification will be central to building authentic relationships and coordinating with the care team to develop personalized recovery plans. Remote work and flexible hours reflect our culture.

Qualificações

• Sustained recovery with at least one year of lived experience (preferred).
• Certification as a Peer Recovery Support Specialist (PRS) or Peer Recovery Coach.
• Strong empathy, listening and communication skills.

Responsabilidades

• Build trusting, nonjudgmental relationships with clients using lived experience.
• Offer hope, motivation, and accountability from personal recovery.
• Collaborate with Holon care team to develop personalized recovery plans.
• Connect clients to housing, healthcare, employment, transportation resources.
• Conduct strengths-based assessments to identify needs and goals.
• Advocate for clients within justice and care systems.
• Partner with Care Navigators, clinicians, and judicial contacts for integrated care.
• Stay informed on best practices in peer support and recovery-based care.
• Maintain confidentiality and HIPAA compliance.
• Contribute to new initiatives and engagement strategies.
