Imagine Pediatrics is seeking a Medication Support Navigator who will join the clinical pharmacy team to improve medication outcomes for children with chronic health and behavioral care needs. This remote role requires coordinating medication access and supporting families.

You will manage medication histories, troubleshoot access barriers with pharmacies and PBMs, and schedule pharmacist-driven programs while working under pharmacist oversight.

Formación

• High school diploma or equivalent.
• Current accredited pharmacy technician certification (PTCB).
• Registration with applicable state boards of pharmacy as required by law.
• 3+ years' experience as a pharmacy technician.
• Pediatrics experience preferred.
• Bilingual English/Spanish highly preferred.
• Ambulatory clinic or specialty pharmacy experience preferred.

Responsabilidades

• Manage, triage, and assign clinical pharmacy referrals to appropriate clinical pharmacy team member.
• Support internal and external team members in the clinical pharmacy referral process based on patient needs.
• Schedule patient/caregiver appointments for pharmacist-driven clinical programs.
• Prepare charts in advance of Clinical Pharmacist appointments.
• Communicates with pharmacies, insurance companies, and internal team members to resolve medication access barriers with the support of pharmacist where needed.
• Completes medication histories.
• Monitor incoming medication renewal requests and manage per internal protocols.
• Monitor incoming prior authorization requests and prepare documentation for review by clinical pharmacist.
• Demonstrates the ability to set priorities, work independently and cooperatively with others within and outside the department.
• Perform other duties as assigned
