Job summary

Clarity Pediatrics, Inc. is hiring a Nurse Care Coordinator (RN) to join an interdisciplinary team and ensure seamless care for children with chronic conditions.

This fully remote role coordinates appointments, educates families, and manages medication authorizations in collaboration with physicians and therapists. The ideal candidate holds an active California RN license, has experience in a remote environment, and can communicate clearly in English and Spanish.

Qualifications

• Active California RN license in good standing.
• Experience coordinating care for pediatric patients preferred.
• Remote work experience and strong organizational skills required.

Responsibilities

• Coordinate clinical appointments across care team.
• Manage referrals to specialists and external services.
• Provide patient and family education on chronic conditions.
• Initiate and track medication prior authorizations with insurers.
• Document interactions across EHR and care management platforms.
• Support care transitions and continuity across disciplines.
