Job summary

Imagine Pediatrics, a tech-enabled pediatric medical group, is seeking a Medication Support Navigator to join the clinical pharmacy team. This remote role supports safe days at home for children with complex health needs and coordinates medication access, histories, and renewals.

You will triage referrals, schedule pharmacist-driven programs, prepare charts, and liaise with pharmacies and insurers under pharmacist oversight.

Qualifications

• 3+ years experience as a pharmacy technician.
• Pediatrics experience preferred.
• Ambulatory clinic or specialty pharmacy experience preferred.
• Proficiency with electronic health record systems.
• Bilingual English/Spanish highly preferred.
• High school diploma or equivalent.

Responsibilities

• Manage, triage, and assign clinical pharmacy referrals to appropriate clinical pharmacy team member.
• Support internal and external team members in the clinical pharmacy referral process based on patient needs.
• Schedule patient/caregiver appointments for pharmacist-driven clinical programs.
• Prepare charts in advance of Clinical Pharmacist appointments.
• Communicates with pharmacies, insurance companies, and internal team members to resolve medication access barriers with the support of pharmacist where needed.
• Completes medication histories.
• Monitor incoming medication renewal requests and manage per internal protocols, where allowable by state law, and seek assistance when outside renewal protocols.
• Monitor incoming prior authorization requests and prepare documentation for review by clinical pharmacist.
• Demonstrates the ability to set priorities, work independently (with pharmacist oversight and review of final product) and cooperatively with others within and outside the department.
• Perform other duties as assigned.

Primary location for this role is remote with quarterly travel for training.

Schedule will rotate to serve families needing later appointment times.

• Central Time hours anticipated being 8:00am-4:30pm CT, 8:30am-5:00pm CT, 9:00am-5:30pm CT, 9:30am-6:00pm CT. May require shifts in EST/PST.
• Eastern Time hours anticipated being 8:00am-4:30pm ET, 8:30am-5:00pm ET, 9:00am-5:30pm ET, 9:30am-6:00pm ET. May require shifts in PST.

Bilingual in Spanish highly preferred for this role.

What You’ll Do

As a Medication Support Navigator at Imagine Pediatrics you will work on the clinical pharmacy team to support our mission of increasing safe days at home for children with special health and behavioral care needs.

As an integrated care team member, the Medication Support Navigator focuses on improving medication related outcomes by supporting medication access needs for pediatric patients with chronic medical and behavioral health conditions. The medication Support Navigator works alongside the Clinical Pharmacist with primary responsibilities including maintenance of patient medication therapy records, accurately capturing medication histories, troubleshooting medication access barriers with community pharmacies and pharmacy benefits managers, and outreach to patient caregivers.

In this role you will:

What You Bring & How You Qualify

• High school diploma or equivalent
• Current accredited pharmacy technician certification (such as PTCB)
  • Maintenance of certification expected
• Registration with applicable state board(s) of pharmacy as required by law
  • Maintenance of all technician registration for all assigned states expected
• 3+ years' experience as a pharmacy technician
• Pediatrics experience preferred
• Ambulatory clinic or specialty pharmacy experience preferred
• Proficiency with electronic health record systems
• Bilingual English/Spanish highly preferred
