Imagine Pediatrics is seeking a Medication Support Navigator to join our clinical pharmacy team. The role focuses on improving pediatric medication access and safety for children with complex needs, working with pharmacists to manage records and outreach to caregivers.

You will triage referrals, schedule pharmacist-led programs, and coordinate with pharmacies and insurers to resolve barriers, while maintaining thorough medication histories and documentation.

Qualificações

• High school diploma or equivalent.
• Current accredited pharmacy technician certification (such as PTCB).
• Registration with applicable state boards of pharmacy as required by law.

Responsabilidades

• Manage, triage, and assign clinical pharmacy referrals to appropriate team member.
• Support internal/external team members in the referral process.
• Schedule patient/caregiver appointments for pharmacist-driven programs.
• Prepare charts in advance of pharmacist appointments.
• Communicate with pharmacies/insurers to resolve medication access barriers.
• Complete medication histories.
• Monitor renewal requests per internal protocols and state law.
• Monitor prior authorization requests and prepare documentation.
