Centene Corporation is seeking a Registered Nurse to assess, plan, and implement complex care management activities for members, developing personalized care/service plans and educating families about benefits to enable high-quality care. The role is remote with a North Carolina residency requirement; RN license required and pediatric CMARC experience preferred.

You will collaborate with providers, monitor plans, and guide new hires, while balancing schedule Monday–Friday 8:00 AM–5:00 PM and

Qualifications

• RN license required.
• 4–6 years of related experience.
• Must reside in North Carolina and be eligible for remote work.
• Pediatric CMARC experience preferred; strong assessment and communication skills.

Responsabilités

• Develops and continuously assesses care plans and collaborates with providers to identify resources.
• Coordinates between member/family and care provider team to ensure person-centered care.
• Monitors plans and progress, revising as needed to meet goals.
• Identifies problems/barriers and suggests interventions for escalated cases.
• Reviews member data to identify trends and improve quality of care.
• Reviews referrals and assessments to develop appropriate care plans.
• Performs outreach via phone/digital/home visits to assess needs and resources.
• Collaborates with healthcare providers to adjust care plans as needed.
