The position requires an active RN license and pediatric CMARC exposure. The ideal candidate will have strong assessment and communication skills, be proficient with Microsoft Office, and collaborate with providers to improve outcomes

Qualifikationen

• Requires an RN license and a Bachelor's in Nursing.
• 4–6 years of related experience in care coordination or CM.
• Experience with pediatric populations (CMARC) is preferred.

• Assess, plan, and implement complex care management activities.
• Develop personalized care/service plans and educate families.
• Coordinate between member, family/caregivers, and care team.
• Monitor plans and revise as needed to meet goals and needs.
• Collaborate with providers to identify resources and address barriers.
