Carolina Complete Health is seeking an experienced Registered Nurse for a remote, North Carolina–based care management role focused on pediatric populations and at-risk children. You will assess, plan, and implement complex care plans, coordinating with providers and families to address unmet needs and ensure high-quality, person-centered care.

The ideal candidate has NC RN licensure, 4–6 years in care management, and CMARC experience.

Qualifications

• RN licensure in North Carolina is required.
• 4–6 years of related care management experience.
• Experience with pediatric population and CMARC preferred.

Responsabilités

• Develops and continuously assesses care plans with providers to address unmet needs.
• Coordinates between member/family and care provider team for person-centered care.
• Monitors plans and member status; revises plans as needed to meet goals.
• Identify barriers for care management and propose interventions.
• Review member data to identify trends and improve quality and regulatory compliance.
• Review referrals and intake assessments to develop care plans.
