Job summary

Imagine Pediatrics is seeking a Care Team Assistant to provide clinical and clerical support in a virtual Value-Based Care environment. The role includes triage calls, lab result communications, and coordinating pre-visit preparations with the care team.

Requirements include high-school level education and 2–4 years of outpatient healthcare experience; Spanish fluency is preferred. Compensation includes benefits, equity program eligibility, and a 3x12 schedule with some travel to Houston.

Qualifications

• High-school level education or equivalency.
• 2-4 years healthcare experience in outpatient clinics; multispecialty clinics strongly preferred.
• Spanish fluency preferred.

Responsibilities

• Accept incoming phone calls and return calls from patients involving clinical inquiries; triage per protocol and route phone notes to the provider for review.
• Place calls and create template-based letters regarding lab results for delivery to patients.
• Review schedule daily to ensure all pre-visit preparations are complete and necessary documents are available in the medical record.
• Call patients at risk of no-show and onboarding patients to remind responsibilities and arrange rescheduling or social referrals.
• Initiate completion of forms for provider review (disability, home health orders, DME, disease status letters).
• Assist care team in regular completion of reports and identify vulnerable patients for referrals.
